Maamwesying North Shore Community Health Services

Who are we?

The Maamwesying North Shore Community Health Services Board oversees both N’Mninoeyaa Aboriginal Health Access Centre and the Baawaating Family Health Team to deliver quality primary and therapeutic health care to all individuals in its eleven First Nation member communities. 

Maamwesying means “the act of working as one” in Ojibway.  The vision of our agency is Maamwesying kina gweyahn N’minobimaadizing which means “Working as One for the Wellbeing of All”.

The following services are available to the First Nation communities in our service area:

Primary Health Care

Our Primary Health Care team provide services to on reserve community members through community based health centres and to the urban aboriginal population residing in Sault Ste. Marie through the Indian Friendship Centre.

Our team is staffed with a Manager, Physiotherapists, Occupational Therapists, Rehabilitation Assistants, First Nation/Aboriginal System Navigators who provide Discharge Planning Services, Assisted Living Services Coordinators, and an Advanced Clinician Practitioner in Arthritis Care (ACPAC) Therapist.

FASD Services

The FASD NEAP Project is a collaboration project in which The N’ Mninoeyaa Aboriginal Health Access Center ( AHAC) is partnered with Health Sciences North, Shkagamik-Kwe Health Centre, Noojmowin Teg Health Centre and Union Of Ontario Indians.NEAP stands for North Eastern Aboriginal FASD Education Partnership Project.

Diabetes Support

Minomodzawin means “To Live Well”. Limited resources are flowed to each of the First Nations to provide health promotion and activities for THE PREVENTION OF DIABETES. This program is supported by the Diabetes Nurse Educator, Registered Dietician, Community CHN and/or Community Diabetes Lay Educator.

Aanjichigewin Health Promotion Program

The focus of the program is on promoting positive health behaviours such as staying active, eating right and staying smoke free, to prevent chronic diseases such as obesity, diabetes, and cancer, increasing individual health throughout the lifespan.

Assisted Living Services

Personal support services, homemaking services, professional services (Traditional Health, Occupational Therapy, Physiotherapy, Dietetics, Social Work, etc.), safety and reassurance checks, emergency urgent response care co-ordination services are available on a scheduled and as-needed basis.

Arthritis Management Program

We offer arthritis management supports including assessments, review of your current symptoms/ limitations review of your medical history and current medication list complete musculoskeletal assessments, and can determine if your arthritis is inflammatory or non-inflammatory.

Rehabilitation Services

We assist clients to reach their full potential.  We work to maintain or restore clients’ current abilities and strengths through a variety of physical programming designed to meet individual needs. Programs such as walking, range of motion (active and passive), and falls prevention encourage clients to stay physically active as long as they are able.

Hospital Discharge Services

The Nurse Case Manager will work with you, your family, and your health providers to ensure that you receive the appropriate services to assist you in living at home with dignity and as independently as possible.

Physiotherapy/Occupational Therapy

We can assess your level of mobility, strength and endurance, diagnose your condition and develop a treatment plan to restore movement and reduce limitations, including pain as well as prescribe and demonstrate specific therapeutic exercises, monitor your progress over time, and adjust your treatment according to your needs and preferences.
